Information Request Form - We appreciate your interest in Chaminade-Madonna College Preparatory and look forward to working with you during your admission process. Please fill out the information request form. This is a request for admission materials and will ensure that invitations for our Open House will be sent to you.

Application Form - This completed application and all other required information most be submitted to the Admissions Office before an applicant is considered for admission.

Entrance Exam Registration Form - For incoming ninth graders only. This form is used to register for the Entrance Exam. All students wishing to be considered for admission into the 9th grade must take the entrance exam. First consideration will be given to students who take the entrance exam at Chaminade-Madonna.
